Cancellation Policy

You are responsible for all charges associated with a visit. If you cancel with less than 12 hours' notice, your credit card on file will be charged $50.  All appointments need to be cancelled within 5 hours of the appointment time or it will be considered a no-show. No-show visits are charged in full and are non-refundable.

Payment Policy

If you have primary insurance that is out-of-network for Spiritual Lactivist, LLC, you must pay the full self-pay fee up front as a deposit. You will not be refunded for any amount either insurance applies to cost-sharing. You will only be refunded if and when Spiritual Lactivist, LLC receives payment directly from insurance(s). Spiritual Lactivist, LLC may keep any amount paid by your insurance(s) over and above the deposit you paid.

An initial visit includes 1 week of follow up support via secure messaging, email, or text. Continued support is available through follow-up visits with insurance or for a fee of $150 per visit (inquire within if accommadations are needed).

A $15 travel fee is added for anyone more than 20 minutes away from me. If your location has a travel fee applied, that fee is not eligible for insurance reimbursement.

You are responsible for verifying your own lactation benefits. Spiritual Lactivist, LLC can only see that you have benefits, I cannot see if you have any special circumstances that might prevent your insurance provider from covering services. If your plan denies coverage of lactation services after the claims have been submitted, you are responsible to pay at the self-pay rate. 

Spiritual Lactivist, LLC may communicate with your insurance company in reference to the services provided to you and your baby or babies.  Spiritual Lactivist, LLC may communicate with your credit card company or bank for any payment related matters.  It is your responsibility to provide accurate and current payment and insurance information. You will update your credit card information as needed and are responsible for any costs and fees associated with your failure to provide updated information.

Payments may be made electronically using a credit card or fund transfer at the time of scheduling.  I use SquareUp to process payments. SquareUp meets the high standards of HIPAA and the banking industry for security and privacy with regard to financial transactions.  However, SquareUp may send, automatically or per your request, email or text message receipts that reveal personal health information such as the date and type of lactation visit.   If you are not comfortable with this, payment may be made via cash or check in advance of your appointment. If payment is not received in advance, your appointment will be canceled.
